DRIVER AUTHORIZATION

DRIVER AUTHORIZATION FORM

In order to drive an Enumclaw School District vehicle, leased vehicle and/or my personal vehicle to transport ESD students, I agree to complete the entire driver authorization process which includes requesting an updated driving record from the Department of licensing in order to determine authorization to transport ESD students to school events, activities and on field trips.

Recommended steps to completing the volunteer drivers authorization form.

Go to the ESD District Website link under Human Relations and click on Volunteers.

1. APPLICATION

  • Click “Register”. 

  • Enter your email address, first and last name and create a password. If you have used our application system in the past, please use the same email and password that you used when you applied.

  • Click "Register".

3. MY PROFILE 

  • Add Additional Names - (Maiden Name, etc.) 

  • Add Birthdate (this is used for background check). The Red Arrows are required information. 

  • Select Interest -  Each interest has different requirements so only choose the interest you will be doing in the immediate future. Click "Next".

  • Add Address, click "Save" and Telephone Number, click "Save". Click "Next".

4. MY CHECKLIST 

  • Click "Volunteer Preferences" - Choose the school where you will be volunteering. 

  • Click "Volunteer Orientation". 

  • Click "REQUIRED TRAINING". 

  • Attest to having reviewed the information. Click "Save".

REQUIRED DOCUMENTS FOR VOLUNTEER DRIVERS 

  • Make separate PDF copies of your driver's license and your proof of insurance card; you must upload these two items separately. Upload documents to the application checklist.

  • Obtain a Driving Record Request (driver abstract) from the Washington State Department of Licensing. The cost is $13.00 and paid by the volunteer. Create a PDF file and upload the document to the application checklist at https://www.dol.wa.gov/licenseexpress.html

You have now completed the necessary steps. You will receive an email from our Human Resources once your background check is complete, notifying you that you are cleared to volunteer.
