This program is available to students who are temporarily unable to attend school for an estimated period of four weeks or more because of physical disability or illness. Tutoring is provided to students who are ill or disabled and require instruction at home or in a hospital. It does not provide tutoring to students caring for an infant or an ill relative.

Services provided are limited to those deemed necessary for temporary intervention as a result of physical disability or illness. They are also limited to a maximum of 18 weeks. Weeks of absences may be consecutive or intermittent, but they may not exceed the 18-week limit. If a student will exceed the 18-week limit, consultation with the Office of Superintendent of Public Instruction (OSPI) health services program specialist is recommended prior to providing extra weeks of tutoring to the student. Tutoring is not provided during school vacations unless students are enrolled in a district summer school program. Home/Hospital tutoring cannot begin for a student if less than four weeks of school remain.

1. A student must be enrolled in the district or must be a private school student receiving ancillary services in the school district.

2. A written statement by a qualified medical practitioner, indicating that the student is unable to attend school due to physical disability or illness for at least four weeks must be on file.
   
   A qualified medical practitioner is a:

• Doctor of Medicine (M.D.)

• Doctor of Osteopathy (0.0.)

• Doctor of Naturopathy (N.D.)

• Doctor of Dentistry (D.M.D.)

• Doctor of Chiropractic (D.C.)

• Physician Assistant (P.A.)

• Advanced Registered Nurse Practitioner (A.RN.P.)

• Certified Nurse Midwife (C.N.M.)
